ABSTRACT

SUMMARY: The study will provide information on the morphometrics of the vertebrae, which can be used to guide clinicians on the appropriate size of transpedicular screws to use in spine interventions among the Jordanian population and for comparative studies with other races. A retrospective analysis of normal CT scans of the lumbar and thoracolumbar areas was done. Linear and angular measurements of 336 vertebrae were collected for 25 males and 23 females. The results were compared between right and left and between both sexes. The L5 has the longest AVBH and the shortest PVBH in both sexes, it also, had the shortest and widest pedicle in both males and females. ratio of the AVBH to PVBH showed progressive increase in both sexes from T11 to L5. Similarly, the VBW increased progressively from the top to the bottom in both sexes, but it was significantly different between both sexes. The L1 was the most cranially oriented vertebrae in males while the L2 showed the most cranial orientation in females. Both sexes L5 was the most caudally oriented vertebrae. This study provides a database for vertebral morphometrics in the Jordanian population, there are slight differences between the right and left side in the upper studied vertebrae (T11-L2) and some measurement showed significant differences between males and females. These findings need to be taken into consideration when inserting pedicle screws.


Este estudio proporciona información sobre la morfometría de las vértebras, la cual puede ser utilizada por los médicos oara determinar el tamaño adecuado de los tornillos transpediculares a utilizar en intervenciones de columna en la población jordana y para estudios comparativos con otras grupos. Se realizó un análisis retrospectivo de tomografías computarizadas normales de las áreas lumbar y toracolumbar. Se recogieron medidas lineales y angulares de 336 vértebras de 25 hombres y 23 mujeres. Los resultados se compararon entre vértebras derechas e izquierdas y entre ambos sexos. La L5 tiene el AVBH más largo y el PVBH más corto en ambos sexos, también tenía el pedículo más corto y más ancho tanto en hombres como en mujeres. La relación de AVBH a PVBH mostró un aumento progresivo en ambos sexos de T11 a L5. De manera similar, el VBW aumentó progresivamente de arriba hacia abajo en ambos sexos, pero fue significativamente diferente entre ambos sexos. La L1 fue la vértebra más orientada cranealmente en los hombres, mientras que la L2 mostró la orientación más craneal en las mujeres. En ambos sexos L5 fue la vértebra más orientada caudalmente. Este estudio proporciona una base de datos para la morfometría vertebral en la población jordana, donde existen ligeras diferencias entre el lado derecho e izquierdo en las vértebras superiores estudiadas (T11-L2). Algunas mediciones mostraron diferencias significativas entre hombres y mujeres. Estos hallazgos deben tenerse en cuenta al insertar tornillos pediculares.

ABSTRACT

Objective@#To determine the effects of lower limb muscle fatigue on spatiotemporal gait parameters and turning difficulty characteristics during the extended Timed Up and Go (extended TUG) test in individuals with different severity stages of Parkinson’s disease (PD). @*Methods@#Forty individuals with PD, classified as Hoehn and Yahr (H&Y) stages 2 and 3 participated in this pre- and post-experimental study design. The participants performed a continuous sit-to-stand task from a chair based on 30 cycles/min set-up to induce lower limb muscle fatigue. They performed extended TUG test immediately before and after completing the fatigue protocol. Spatiotemporal gait parameters and turning difficulty characteristics were recorded using two GoPro® Hero 4 Silver cameras. Data were subjected to a repeated-measure ANOVA. @*Results@#Individuals with PD experience significant changes in spatiotemporal gait parameters, specifically stride velocity and length, under conditions of lower limb muscle fatigue (p=0.001). These changes were more pronounced in individuals with PD in the H&Y stage 3 group. Additionally, both PD groups exhibited difficulty with turning, requiring more than five steps to complete a 180° turn and taking more than 3 seconds to accomplish it. @*Conclusion@#These findings highlight the impact of muscle fatigue on gait performance in PD and suggest that individuals in later stages of the disease may be particularly affected. Further research is needed to explore interventions that can mitigate these gait impairments and improve mobility in individuals with PD.

ABSTRACT

@#Discordant lymphoma (DL) is the coexistence of two or more distinct subtypes in separate anatomic sites. There are limited reports on DL cases especially involving more than two subtypes in more than two sites. We report a 76-year-old man who presented with constitutional symptoms, flank mass and painless lymphadenopathies for six months. Laboratory tests revealed moderate anaemia, markedly elevated serum IgM (13400 mg/dL), IgM Lambda paraproteinemia and Lambda light chain paraproteinuria with unmeasurable serum lactate dehydrogenase due to hyperviscous sample. CT scan showed multiple subcutaneous masses over chest wall and retroperitoneum, with lytic bone lesions, and hepatosplenomegaly. Further biopsy findings with morphological, immunohistochemical and molecular analysis of the tissue sections revealed diffuse large B-Cell lymphoma in the chest wall mass, follicular lymphoma in the inguinal lymph node and lymphoplasmacytic lymphoma in the bone marrow. This case highlights the rare DL. The importance of histopathological evaluation of lymphoma despite the availability of PET-CT scans for disease staging is undeniable.

@#Introduction: Daily use and improper management of plastic materials by the public contributes to the occurrence of microplastic pollution. It can be acknowledged that knowledge and attitudes are one of the keys to reducing the release of microplastic into the environment. Therefore, enhancing student’s understanding may play an important role in effective solution innovation and readiness to take specific actions for environmental protection. Method: This cross-sectional study was conducted among university students using an online questionnaire to assess their knowledge and attitudes about microplastic pollution. For data analysis, Pearson’s Chi-square was conducted using SPSS version 26. Results: Findings revealed that students have moderate knowledge (4.66) and attitudes (5.42) towards microplastic pollution. However, neither age, gender, CGPA, nor the course in which they studied had any significant association with their level of knowledge and attitudes (p>0.05). In fact, the level of knowledge and attitude reported by respondents indicate that it is necessary to address this deficiency by providing education through structured education and mass media, along with vigorous policy enforcement and replacement of conventional plastics. Conclusion: Education on microplastics among the younger generation is key to curbing the problem of microplastic pollution. As future consumers, the demand for alternative products that does not contribute to microplastic can help encourage brands and markets to come up with better solution that is critical to prevent this problem from worsening.

ABSTRACT Objective: The objective of this article was to evaluate the effect of alpha binaural beat music on pain level after initial placement of a maxillary fixed appliance, compared to music without binaural beats (placebo) and no music (control). Methods: 60 patients undergoing maxillary fixed orthodontic appliance and initial archwire placement were randomly allocated into the three aforementioned groups. The pain level experienced was monitored for the following seven days, using the short-form McGill pain questionnaire (SF-MPQ). Results / Descriptors: Intensity of both sensory and psychological aspects of pain reduced significantly in the binaural beat music (BBM) group, compared to the control, after the 5th day. Statistically significant lower affective and total pain scores were also found on day 6 for the placebo group, compared to the control. Present Pain Intensity (PPI): Statistically significant lower scores were found between the BBM group and the control group from days 3 to 7. Statistically significant lower scores were also found between the placebo and the control groups on days 4, 5 and 6. Visual Analog Scale (VAS): Compared to the control group, the placebo group had a lower VAS score on day 4, and the BBM group had lower scores on days 6 and 7. Conclusions: There was a significant reduction of pain demonstrated in the BBM group, compared to the control, toward the end of the first week of treatment. There was no difference in reported pain between the BBM and placebo groups for any of the scores.


RESUMO Objetivo: O objetivo deste artigo foi avaliar o efeito da música alfa com ritmo binaural sobre o nível de dor após a instalação de um aparelho fixo superior com arco inicial, em comparação com a música sem ritmos binaurais (placebo) e sem música (controle). Métodos: 60 pacientes submetidos a instalação de aparelho ortodôntico fixo superior e do arco inicial foram alocados aleatoriamente nos três grupos acima mencionados. O nível de dor experimentado foi monitorado durante os sete dias seguintes, usando a forma curta do questionário de dor McGill (SF-MPQ). Resultados: Após o quinto dia, a intensidade dos aspectos sensoriais e psicológicos da dor foi significativamente reduzida no grupo de música com ritmo binaural (BBM), em comparação com o grupo controle. Escores significativamente menores de dor afetiva e total também foram encontrados no dia 6 para o grupo placebo, comparado ao controle. Intensidade da Dor Presente (PPI): escores significativamente menores foram encontrados para o grupo BBM, em comparação ao grupo controle, nos dias 3 a 7. Escores significativamente menores também foram encontrados para o grupo placebo, em comparação ao grupo controle, nos dias 4, 5 e 6. Escala Visual Analógica (VAS): comparado ao grupo controle, o grupo placebo teve um escore VAS menor no dia 4, e o grupo BBM teve escores menores nos dias 6 e 7. Conclusões: No final da primeira semana de tratamento, houve redução significativa da dor no grupo BBM, em comparação ao grupo controle. Não houve diferença entre a dor relatada nos grupos BBM e placebo para qualquer um dos escores.

@#The present study compares the in vitro effects of nanoparticles loaded pentamidine drug and conventional pentamidine on Leishmania tropica. Herein, pentamidine-loaded chitosan nanoparticles (PTN-CNPs) have been synthesized through an ionic gelation method with sodium tripolyphosphate (TPP). Next, the physical characteristics of PTN-CNPs were determined through the surface texture, zeta potential, in vitro drug release, drug loading content (DLC), and encapsulation efficacy (EE) and compared its efficacy with free pentamidine (PTN) drug against promastigotes and axenic amastigotes forms of L. tropica in vitro. The PTN-CNPs displayed a spherical shape having a size of 88 nm, an almost negative surface charge (-3.09 mV), EE for PTN entrapment of 86%, and in vitro drug release of 92% after 36 h. In vitro antileishmanial activity of PTN-CNPs and free PTN was performed against Leishmania tropica KWH23 promastigote and axenic amastigote using 3-(4, 5- dimethylthiazol-2-yl)-2, 5-diphenyletetrazolium bromide (MTT) assay. It was observed that the effect of PTN-CNPs and free PTN on both forms of the parasite was dose and time dependent. Free PTN presented low efficacy even at higher dose (40 µg/ml) with 25.6 ± 1.3 and 26.5 ±1.4 mean viability rate of the promastigotes and axenic amastigotes, respectively after 72 hrs incubation. While PTN-CNPs showed strong antileishmanial effects on both forms of parasite with 16 ± 0.4 and 19 ± 0.7 mean viability rate at the same higher concentration (40 µg/ml) after 72 hrs incubation. Half maximal inhibitory concentration (IC50) values of PTN-CNPs toward promastigotes and amastigotes were obtained as 0.1375 µg/ml and 0.1910 µg/ml, respectively. In conclusion, PTN-CNPs effectively inhibited both forms of the L. tropica; however, its effect was more salient on promastigotes. This data indicates that the PTN-CNPs act as a target drug delivery system. However, further research is needed to support its efficacy in animal and human CL.

@#Introduction: : Current tuberculosis (TB) eradication strategies have shifted away from the medical sphere, focusing on a more holistic approach that encompasses both healthcare and social support. This means a greater emphasis on TB prevention strategies, particularly on social determinants of TB. Using a locality in Malaysia as a case study, a research was carried out among 844 TB cases in Pasir Mas, Kelantan (2013 to 2017) to identify the socioeconomic distributions of TB defaulters and its implications for current TB prevention strategies. Methods: TB patients’ sociodemographic characteristics and treatment outcomes were extracted and analyzed from the Malaysian registry. Pearson’s chi-square test was used to determine sociodemographic factors associated with TB defaulters. Results: Gender, age and education levels were significantly associated with default treatment (p<0.05), highlighting the need to focus on adult male patients with low educational background. Conclusion: Results indicate that current national TB management needs to focus on targeting those at increased risk of defaulting by understanding gender-specific challenges to treatment. While more research is needed to explore the gender associated issues related to treatment defaulting, we are also calling for a change in current TB management practices to one that focuses on gender-specific intervention that addresses personal and societal challenges to TB treatment.

@#Introduction: Food insecurity is currently a problem faced by both developing and developed nations. Issues of food insecurity reflect both social and economic challenges, posing a significant policy dilemma. Although there has been various research on food insecurity, very little has been done to better understand what are the sociodemographic factors that can predict food insecurity within a Malaysian context, especially amongst the unique community of Malay university students, which makes up the largest group of students in tertiary education. Therefore, this study examined the prevalence of food insecurity and its associated factors among Malay university students. Methods: Conducted in Universiti Teknologi MARA, Selangor Campus, conveniently sampled participants (n = 181) completed a pre-tested English version of the US Adult Food Security Survey Module (AFSSM) questions in 2015, which measures their level of food security status. Binary logistic regression analysis was carried out to determine the sociodemographic predictors of food insecurity among the sampled population. Results: More than one-third of participants (42.5%) reported food insecurity within the past year with 22.1% experiencing very low food insecurity. Additionally, of those who experienced food security, 5.4% reported a low food security status. However, no significant predictors were observed positively related to food insecurity. Conclusion: Data indicates that food insecurity is a significant problem among university students and additional research is needed to better understand and address food insecurity in higher education.

Aims@#The primary aim of this study was to utilize abundant palm oil mill effluent (POME) waste and turn it into a value-added product of biomass fuel with high calorific energy value (CEV) via fermentation and drying process, then simultaneously reduce abundant liquid waste.@*Methodology and results@#POME is available abundantly in Malaysia and only a small portion of it is utilized to produce other value-added products. In this study, fermentation of POME in the presence of bacteria (Lysinibacillus sp.) and fungus (Aspergillus flavus) separately at 37 °C, 180 rpm for 5 days, followed by overnight oven-drying at 85 °C was conducted. Four fermentation medium conditions were performed, viz.: (1) autoclaved POME, (2) autoclaved POME with the addition of Lysinibacillus sp., (3) autoclaved POME with the addition of A. flavus and (4) POME as it is (non-sterile).@*Conclusion, significance and impact of study@#Among all conditions, fermentation utilizing autoclaved POME in the presence of A. flavus evinced the highest CEV of 25.18 MJ/kg. The fermentation in the presence of Lysinibacillus sp. strain revealed high COD and BOD removal efficiency of 59.20% and 320.44 mg/L as well as the highest reduction of oils and grease among other groups with the value of 15.84%. Future research directions are proposed for the elucidation of co-fermentation in the presence of both Lysinibacillus sp. and A. flavus.

ABSTRACT

Solitary fibrous tumor (SFT) is a rare type of mesenchymal neoplasm that first was discovered in the pleura but can also affect the peritoneum, lungs, mediastinum, and skin. Cutaneous malignant SFT is an extremely rare tumor that resembles dermatofibrosacoma protuberance (DFSP) histologically and immunohistochemically. Herein, we describe a case of malignant SFT that presented as a recurrent mass on the scalp. The first lesion was totally excised one year before recurrence and was diagnosed as a DFSP based on the histopathology and cluster of differentiation 34 immunostaining positivity. Re-examination of the previously examined specimen was considered. Activator of transcription 6 positivity was also detected in the tissue, confirming the diagnosis of a recurrent malignant SFT rather than DFSP. There was no evidence of recurrence, locoregional, or distant metastases at six months after lesion removal with a safety margin.

@#Introduction: Dental surgeon exposes to numerous occupational hazards include the bloodborne pathogens, pharmaceuticals, ergonomic hazards, noise, vibration and workplace violence. It causes prolonged health effects on short or long term exposure include musculoskeletal disorder, back pain and other health issues. This study aimed to identify the awareness level of occupational hazards and experiences among dental surgeons and to determine whether preventive measures are employed. Methods: A cross-sectional study design and convenience sampling method had been used in this study. The Occupational Hazards in Dentistry questionnaire was self-administered by 39 dental surgeons in International Islamic University Malaysia, Kuantan. The data were analysed using descriptive, chi-square and thematic analysis. Results: The overall response rate was good (78%). Among the participants, 59% were female, and 41% were male. Half of the participants (59%) had worked for 11-20 years. The majority of the participants (82%) practised a correct posture while providing dental treatment. The most-reported pain areas were at the shoulder and neck and followed by the lower back. Approximately, 10% of the participants had been diagnosed with cervical spondylitis/ degenerative cervical discs. Most participants have taken preventive measures to reduce exposure to these occupational hazards. Conclusion: Work-related pain is common among dental surgeons. The most affected pain area is the shoulder and neck, followed by back and wrist pain. Occupational hazards can be alleviated effectively using a comprehensive approach. It is essential for dentistry to incorporate comprehensive strategies into practice to improve productivity and enhance occupational safety.

Human telomerase reverse transcriptase (h-TERT) protein resumption is the main process of preservation of telomeres that maintains DNA integrity. The present study aims to evaluate the prognostic role of ALK-1 and h-TERT protein expression and their correlation with ALK gene alterations in glioblastoma multiforme (GBM). @*Methods@#The current study is a retrospective study on a cohort of patients with GBM (n = 53) that attempted to detect ALK gene alterations using fluorescence in situ hybridization. ALK-1 and h-TERT proteins were evaluated using immunohistochemistry. @*Results@#Score 3 ALK-1 expression was significantly associated with male sex, tumor multiplicity, Ki labeling index (Ki LI), and type of therapeutic modality. Score 3 h-TERT expression exhibited a significant association with Ki LI. ALK gene amplifications (ALK-A) were significantly associated with increased Ki LI and therapeutic modalities. Score 3 ALK-1 protein expression, score 3 h-TERT protein expression, and ALK-A were associated with poor overall survival (OS) and progression-free survival (PFS). Multivariate analysis for OS revealed that ALK gene alterations were an independent prognostic factor for OS and PFS. @*Conclusions@#High protein expression of both ALK-1 and h-TERT, as well as ALK-A had a poor impact on the prognosis of GBM. Further studies are needed to establish the underlying mechanisms.

A case-control 3:1 study was performed at the outpatient clinic. Direct stool examinations were done. One hundred sixty patients were included, 40 cases and 120 controls. Positivity to Blastocystis sp. was associated with dyspepsia (P <0.001), bloating (P <0.001) and abdominal pain (P = 0.03) in patients attending our hospital outpatient clinic.

This study aims to evaluate the chemopreventive activity of two antioxidants (curcumin [CUM] and caffeic acid [CAF]), focusing on how these antioxidants could reduce cytotoxicity induced by short term secondhand exposure of waterpipe tobacco smoking. Forty-eight adult male BALB/c albino mice were equally divided into four groups. Antioxidants were delivered intraperitoneally, and the exposure to waterpipe smoking (WPS) was performed using a smoking machine. This experiment lasts for 14 consecutive days. Serum were collected from mice before dissection to quantify the activity of some liver enzymes, kidney function tests and proinflammatory cytokines. Lung, heart, and kidney were isolated and processed for light microscopy technique. Parallel treatment of CUM or CAF along with exposure to WPS showed less inflammation, less vacuolized, and more inflated alveoli, less deteriorations in cortex part of kidney, and less disintegration of cardiac myofibers in comparison to waterpipe only. Besides, CUM and CAF significantly reduced the activity of aspartate aminotransferase and proinflammatory cytokines. CUM and CAF were found to have anti-inflammatory and ameliorative effects against the cytotoxicity induced by exposure to waterpipe tobacco smoking, and CUM showed better chemopreventive activity than CAF.

Parallel treatment of CUM or CAF along with exposure to WPS showed less inflammation, less vacuolized, and more inflated alveoli, less deteriorations in cortex part of kidney, and less disintegration of cardiac myofibers in comparison to waterpipe only. Besides, CUM and CAF significantly reduced the activity of aspartate aminotransferase and proinflammatory cytokines. CUM and CAF were found to have anti-inflammatory and ameliorative effects against the cytotoxicity induced by exposure to waterpipe tobacco smoking, and CUM showed better chemopreventive activity than CAF.

RESUMEN Los hongos formadores de micorrizas arbusculares (HFMA) son simbiontes obligados presentes en la rizósfera de plantas de cacao y la diversidad de sus comunidades se modifica, dependiendo de diversos factores como la presencia de cadmio (Cd) en el suelo. La persistencia de HFMA en suelos enriquecidos naturalmente con Cd podría ser un indicador de su capacidad para tolerar esta condición. Esta investigación caracterizó la estructura de la comunidad de HFMA locales presentes en la rizósfera de plantas de cacao en dos suelos con baja (B-Cd: 0,1 mg kg-1) y alta (A-Cd: 20,9 mg kg-1) concentración de Cd. Esporas de HFMA se identificaron mediante claves taxonómicas y su abundancia, riqueza y diversidad se determinó en muestras de suelo originales y después de su multiplicación mediante cultivos trampa. Las comunidades de HFMA se compararon usando análisis de componentes principales (ACP) e índices de diversidad alfa y beta. Los resultados indican que A-Cd presentó valores significativamente menores de abundancia (21 %), riqueza (20 %) y diversidad (11 %) de morfoespecies de HFMA con respecto a B-Cd. Las dos comunidades de HFMA presentaron cinco de siete géneros en común, pero solo cuatro de las 23 morfoespecies descritas se encontraron en ambas comunidades. El análisis de diversidad beta y el ACP determinaron baja similaridad y tasa de recambio entre las comunidades de HFMA. La dominancia de Diversispora spurca, Rhizoglomus sp. y Claroideoglomus etunicatum en A-Cd sugiere que estas morfoespecies son estrés-tolerantes y candidatos potenciales para el desarrollo de estrategias de mitigación en suelos con Cd.


ABSTRACT Arbuscular mycorrhizae fungi (AMF) are obligate symbionts present in rhizosphere of cocoa plants and their community diversity is modified depending on several factors, such as cadmium (Cd) presence in soil. AMF persistence on Cd natural enriched soils might be an indicator of their tolerance and their potential in biotechnological applications. In this research we characterized local AMF community structure present in cocoa rhizosphere soils with low (B-Cd: 0.1 mg kg-1) and high (A-Cd: 20.9 mg kg-1) natural Cd concentrations. AMF spore identification was carried out using taxonomic keys and their abundance, richness and diversity were determined in original samples and after multiplication process using onion trap cultures. AMF communities were compared using alpha and beta diversity indexes and principal component analysis (PCA). The results indicated that A-Cd presented significative lower values of abundance (21 %), richness (20 %) and diversity (11 %) of AMF morphospecies in comparison with B-Cd. Both AMF communities presented five of seven genera in common, but only four of 23 morphospecies described were found in two communities. Low similarity and turnover were found among AMF communities throughout beta diversity analysis and PCA. Dominance of Diversispora spurca, Rhizoglomus sp. and Claroideoglomus etunicatum in A-Cd suggests that these morphospecies are stress-tolerant and they are potential candidates for the development of mitigation strategies in cocoa plants under Cd stress.

ABSTRACT

Objective: This was the first national epidemiological study on oppositional defiant disorder (ODD) in Iran, which provided new information about the prevalence, comorbidities, and sociodemographic predictors of ODD. Methods: Data from a face-to-face household survey of 30,532 children and adolescents aged 6-18 years were collected from across all 31 provinces of Iran using a multistage cluster sampling design. The Persian version of the Kiddie Schedule for Affective Disorders and Schizophrenia for School-Age Children - Present and Lifetime Version (K-SADS-PL) was used in this study. Results: The lifetime prevalence of ODD was found to be 3.9%. ODD was significantly more common in boys than girls and appeared in late adolescence more frequently than in childhood. A lower prevalence of ODD was found among participants who lived in rural areas. ODD is highly likely to co-occur with attention deficit hyperactivity disorder, separation anxiety disorder, generalized anxiety disorder, and depressive disorders. Conclusions: The findings of this national population-based study confirm and extend previous findings on the prevalence, comorbidities, and sociodemographic predictors of ODD.
